should I include in my home file?
- Basic information about you or your family member so that others can give consistent care.
- A list of all the people, agencies, programs and organizations that are providing services to
you or your family member.
- A list of the chain of command within any agency or organization you work with, including service providers/programs and local, state and federal agencies. Include addresses and telephone numbers for easy reference.
- Copies of test results and recommendations from independent assessments.
- All written including handwritten letters and notes to and from all professionals working with you or your family member.
- All written communication with outside professionals regarding you or your family member's unique needs.
- A list of medications being taken at home, school, or work as authorized by the physician. Include the kind of medication, dosage, and time of day taken. In addition, note the prescription number, as well as any changes in dosage or reaction.
How
should I keep my records organized?
- Keep records chronologically, with the most recent on top.
- Date any notes you take about meetings or phone conversations with physicians and other professionals handling your or your family member's needs.
What
records should I take to important meetings and appointments?
We recommend that you keep a notebook containing your most recent records which can be easily accessed to take with you to important meetings and appointments. This notebook should include at least two years of reports, evaluations, program plans and any documents that pertain to you or your family member.
