Welcome to GGRC’s online referral form. It may be helpful for you to gather copies of any school records, medical records or evaluations related to the Applicant before getting started. These records may help in answering questions. We will also ask for you to upload certain records if appropriate.

GGRC is only able to serve individuals living in San Francisco, San Mateo, and Marin Counties. If you live outside of one of these counties, please visit the Department of Developmental Services website and use the Regional Center Lookup to find the regional center that serves your county.
